Of course most of us do not "literally" accept the vows. "Worse" could include snoring, but it could include serial killing, abuse, molestation - pretty much everyone agrees then that we did not agree to that... Preachers used to say "do you promise to love honor and obey." Since that was found to be politically incorrect (and stupid) to only have the woman swear to obey, marriages changed to both swearing to obey each other. Since that was as stupid an idea as it was inpractable, its hard to find a wedding ceremony with obey anymore. I take the cartoon to mean she does not literally agree to a lifetime commitment.
